So... as Sayaka shows us, Witch-outs are the result of a meguca not being able to accept their lesbianism. Sayaka lasted so little because she was convinced she was crushing on a boy... and then Koko and to come and be all super awesome and cute and tsunsun derere and totally steal Sayaka's heart before she was ready to break free from the oppressing norms of society, and accept her true nature as a Magical Girl.
Really, it's all Koko's fault for being so fabulous.

Obviously, it wasn't Kyouko's fault, but Homura's. As Homura went through the loops, she became more fabulous and Sayaka became more antagonistic towards her. Homura's dynamic entrance during her fight with Kyouko didn't help either.
