Settra is actually very pious and does not consider himself a god or above them or even equal to them. I'm pretty sure he only acknowledges the gods of Nehekara as being above him and the only one of them still alive is ironically Usurian the God of the Dead, so Setra thinks he is superior to every existent god but that one.

That does make something of an ironic twist, the steward of Karak Eight Peaks commanded the reclamation of a Karag, whereas the martial will never get the chance.
Maybe it has to do with her being sent off to woo Belegar and getting distracted with Prince Kazrik.

Not really an obstacle to him. Settra went to Norsca in the War of Sand and Snow because a Norscan stole his crown.

I honestly wouldn't be surprised if the only reason Settra isn't a God is he took a look at the other Gods around and decided actually ruling stuff on the material plane seems pretty difficult when you are one.
Pretty sure Sigmar would have made some different decisions some time in the last few millennia if he was still in charge.
