While it can safely be assumed that Medea is always in the middle of some good low level plotting I think the main plotter here is Circe.

As for the Sakura thing... well in character it isn't like Shirou knows she's a potential bomb about to go off beyond maybe a vague impression that her home life sucks (I mean living with Shinji has got to be a red flag if nothing else) so everything else has kinda crowded her out.
