If memory serves me right the Italian Nationalists didn't really become a political force until after Napoleon had been defeated. Mostly because their leadership was made up of people who had benefited from Napoleon's Republic/Kingdom of Italy (basically North Italy reorganised 1802-1814) and after temporarily going back to the old order of city states realised that they had it better under Napoleon.
I think the concern with Italian Nationalism is a) it would create a powerful state that might be too powerful to be a sister republic and b) France has what some might call Italian land under its direct jurisdiction and if I know anything it's that people had loosing land.

Honestly taking Sicily will up our reputation. Can't remember if this is right but wasn't Sicily not taken in the original timeline? Then there is France getting the benefits of the location of Sicily. Actually how will France government and people react to us taking Sicily the way we did?

Not that it's safe for us as it is but this would be massively terrifying if Napoleon and our Brother weren't in power right now because our popularity would terrify our enemies. I feel like Egypt would become the Gaul to our Caesar in that situation.
