Why are you surprised? Not only did he get turned into a beast and end up in the hunter's nightmare but he also was using an Old One artifact for who knows how long, to the point that he calls it his mentor. Although it is interesting how he still retains some semblance of his humanity.
I think I'm more surprised about the extent of his horribleness, the overall battered appearance, the multiple stunted-limbs sticking out at odd angles, two heads, what appears to be a wolfs head growing on top of his own deformed head, both wolf mouths positivity filled with eyes....
It's the most extreme case of the Old Bloods mutations we've seen so far.
