You only remember something like a holy light after killing Berserker and dying; Saber most likely did something, but it apparently was too late for you. You eventually notice that you feel like you're floating in something warm. You decide to contemplate how, exactly, you died; something feels off about that.
You consider the moments after Berserker came back to life. You feel yourself shiver at the phantom sensation of being crushed. Then, Shirou used-
Wait, Shirou? Wasn't Saber the one who freed you?
Ca- urn
What? You feel even more confused now. What is it you're hearing and seeing? Didn't you die when Berserker squeezed you? If so, how did you end up here? Are you even dead?
Unlocked the self-imposed mission 'What's in a Memory?'
Ah, you are tired of thinking about that! It's a rather traumatic memory, and fresh in your mind. (No, it isn't that traumatic. You are made of sterner stuff than that!)
Before you even realize what's happening, you're someplace cold. Giant hands are holding you, you can't control your body, and you are panicking despite yourself. You really don't like not being in control of your body; after a bit of time of the sensation, you got so frustrated that you started letting out some vile curse words. In German and English.
Or, at least, you tried. For some reason, you can't control your mouth either! After a while of trying to curse, you hear something that sounds like an infant or small baby crying, but ignore it in favor of continuing your tirade.
You narrowly dodged being considered a possessed baby due to serious luck. This action is also rather in-character, which pleases the QM. Two permanent half-steps in luck (2x (+) = (++)) Congratulations, Luck E -> D-, you now have the luck of the average normal person from your previous universe
Due to taking your first breath, you have unlocked the Traits, Perks and Flaws system.
(Unlocking the Traits system would have happened regardless of the choice made)
Thanks to the disconnect of body and spirit caused by you failing to control your body, Trait 'Shaky Connection - Mild' has been discovered and upgraded to 'Shaky Connection - Moderate'
+This trait means that you sometimes cannot control your body at random moments.
+At higher levels, it might cause narcolepsy or even a coma. At too-low levels, it may lead to you losing memories from your past life.
You hear a few voices, but are having a hard time catching what they are saying. What kind of dialect are they using? You are certain it's Japanese, but it's subtly… off.
"Congratulations Uchiha-san, it's a girl," a voice says, and you want to tell that voice that 'of course I'm a girl, thank you very much'. You keep your mouth shut, though. You're getting suspicious right about now, and have formulated a hypothesis (or three) on what's happening. You dearly hope you are wrong, though, damn it!
Your vision clears a bit, but you still can't see very well, nor can you control your body. You are given to a person lying down, and can see strands of red. The person pulls you closer, and you get a glimpse at a woman's face, and eyes whose color is… Red? Are they a Dead Apostle or something? No, that can't be right.
Your mother's eyes are red; she is very closely related to ??? You gain Trait 'Closely related to (???)'
Gives you the Perk 'Super Natural Sensor'
(+Start with Chakra-Sensor Skill (Genuine), at C+, training gains in the sensory skill are greatly-colossally faster),
the Perk 'Bite me, bite me'
(+Colossally increases the effectiveness of the ???? Part of the Uzumaki clan's heritage.),
the Perk 'My Chains! - Slight-Mild'
(+Slightly-mildly increases the effectiveness of training the ?? Chains)
and the Flaw 'Losing my Light'
(+When/if you get the ?????, using its techniques will blind you faster)
Gained a (+) in the Chakra Control skill, to be applied when you unlock it, due to your relation to ???
Another giant, one who sounds like a boy just entering puberty, enters the room. If it is, in fact, a room. He (?) is told something about a sibling being born. He is addressed as Shisui. Despite yourself, you feel your body shiver; you can tell that this boy is somehow very important, but you can't tell why, exactly, that is so.
You gain Trait 'Shisui's sister'
Gives Perk 'Let's Flicker'
(+A huge bonus to everything related to reflexes and all other aspects of speed
+A mild bonus to training related to the Wind element),
Perk 'They have Mental Interference here, too?'
+A moderate bonus to all Genjutsu and its defense
+A moderate bonus to mind-controlling / suggestion skills
+You were relatively bad at Mental Interference Magecraft, so don't expect miracles
+No, you don't get Koto---
And Perk 'Sister complex ahoy!'
(+Your reputation with Uchiha Shisui starts at D-, goes up Beyond Colossally faster, and goes down greatly more slowly.)
You are handed over to the boy and as that happens, the truth dawns on you: you are a baby. You've already been reborn. Oh crap. At least you have your memories for some reason. Your brother (?) is asked to name you, and a moment of dread takes you over before the boy speaks.
He says, "Rin. This child's name will be Uchiha Rin," officiously, and it's all you can do not to laugh at his serious (seriously cute!) act.
You didn't expect to have the same given name as your previous life, though. Either you got very, very lucky (which is insanely rare for you), or something else is afoot.
You start laughing at the irony, and keep laughing as Shisui is making strange sounds - is he trying to entertain you? You snuggle into him (what? He's warm!) and everything goes dark.
Your reputation with Uchiha Shisui goes up. Rep: Shisui: D- -> C+ rank
You find yourself on a strange platform, seemingly made of glass, and something that looks like a Rune adorning its surface. You look around, and beyond the lit platform, everything is dark. You are back in your body, that of Tohsaka Rin, that is, though you think you are a bit taller than before…? Or maybe you got that impression because you were a baby for a little while.
Speaking of which, was that a dream? Or are you dreaming now? You examine the Rune again, and you somehow know that it means 'Origin', though it doesn't belong to any Runic system you recognize.
Suddenly, there are three podiums in front of you, each holding an item. 'Where did these come from?' You think, a bit spooked.
One holds a sword, and somehow reminds you of Shirou; one holds a replica of the Caduceus, and reminds you of Kirei slightly. The final one is a convincing replica (?) of a particular gem you remember very well - it's the one you used to revive Shirou that one time.
You get closer to the podiums, then

+You gain the Trait 'If you aren't cheating, you're not trying'
After you make your choice, you end up… in a giant crib!? No, it's a normal one, if larger than average. You're just… small. So, it wasn't a dream, after all?
You are not in a hospital anymore, and your eyesight is much improved, as is your motor control. For all you know, months might have passed. Before you know it, you are picked up by a redheaded woman, the one who is probably your mother. She bares her breast, and you squirm in place, knowing what is going to happen.
You-

When your mother (?) is finished with trying to feed you, she gently rocks you, but you don't think you can sleep yet.

Anything 'considered' a Demon. So, because of the general public's perception, Bijuu count as demons for the purpose of this, the same with Kaguya. Though since Kaguya isn't remembered in modern times, the bonus will probably have a serious malus.

I say if we pick that origin a slight focus on physical stats would be necessary. Seeing as its a reincarnated Rin who's a genius by the way, picking up chakra stuff would be slightly easier for her than normal. Chakra is based on a system in the body, just like magic circuits too.

I mean, you're not wrong about her being a genius, but that's like saying a nuclear physicist should know astrophysics. There's a definite flaw in your reasoning.

Just because we've got an extra circulatory system in addition to / instead of an extra nervous system doesn't mean we won't pick up supernatural abilities (like using Chakra) faster.

Secret option woohoo! "If you aren't cheating, you aren't trying" screams "ninja" too much not to take. @AoMythology does the malus to support skills include self-supporting skills, like physical conditioning etc.? And if not, how strictly is "support" defined? I assume it's meant strictly for supporting others, but tracking etc. could feasibly fall under that definition.

No need to deprive ourself of our mother's milk - it's good for you!

I'm picking this because I half-suspect it'll be harder if we leave it for later.
Cheat the system... Does not actually sounds all that great, tbh.

I specifically pointed out that she's a genius, but, using the QM's thing, you're saying a Cardiologist could totally learn to be a Neurologist without any issue.

Ninja literally have tests on how well they can cheat on written exams, so this is both fluffy and a no-brainer.

If it isn't a combat support skill, it doesn't suffer the malus. Tracking doesn't get nerfed. Genjutsu is nerfed in training, but in combat, it's not affected if you're fighting alone. Conditioning is not affected at all.

That's because support skills are rather vital for a team. Basically, this option won't serve you well in a cell, especially if we get Kakashi as a teacher. The "great bonus to all training of offensive skills" IS rather more OP than it sounds, though, I think.
If any of you wants Rin as the medic in a team (which is definitely possible, esp. with the healing bite thing), well... you know what not to pick.
Edit: I can't believe it, but I forgot about Shisui's suggestion genjutsu. Editing it in now.
Not actually a huge fan of the hidden option. Archer is cool and all, but I'd rather have Rin stay true to her roots - not to mention, it'd be nice to explore gem magic a bit in a quest. And while it doesn't have a built-in buff to frontline combat, but there's nothing preventing us to get good in it anyway - Rin wasn't exactly helpless in that area (just surrounded by freaks), and having ninja training on top of her old speciality should provide interesting opportunities.

Gem is the actual mystery box here even if it wasn't in hidden text. The Archer option doesn't have ???? and isn't about incredibly exotic shenanigans.
Besides this is supposed to be Rin, not some person who remembers being her. I want to keep being that fairly good natured Tsundere.
Gem Magecraft isn't affected by the (Origin) vote; we might not even have Circuits anymore to use it.
Added to Shisui-given perks; I can't believe I forgot.
