Actually Oedon Chapel is protected by Oedon, the formless, shapeless Old One. It's more than just incense keeping evil at bay. Nothing really wants to mess with Oedon- even if it can't affect the world directly, it still seems to be able to influence it through the control of blood.

Yeah. You need to have a blood-link to a Great One for them to be able to impregnate you. Iosefka might, but it sounded like she was performing some kind of weird alchemy on normal blood to create her vials. The two girls are probably safe though- we've no sign that they're at all tied to the Great Ones in-game.

A Iosefka's Blood Vial can be found immediately before Murgo's Blood Nurse. I'm not sure she doesn't have a connection.
