When I mean everyone in prison will roll for suicide/sanity... I mean everyone... All two hundred people will roll and you will see the damage you will cause first hand.

They have a +50 to that roll, so unless they roll a nat 1, you won't see them dying or becoming vegetables.
No... You just might get the trauma roll of becoming a mass murderer of children who are not your enemy.

So we have like 10 friends in the danger zone for it?
Probability of none of them rolling a Nat 1 is about 9/10 then. Definitely a risk still, but loosing them is unlikely.
Unfortunately we're basically guaranteed to kill a lot of people even if the roll to beat is very low -- rolling too many dice to do anything else.
I would say give anyone with a name some plot armor for it though -- it makes no sense of mooks to survive where a Named gets wrecked in Naruto
