Okay, so as discussed before, I am going to give you all a chance to help me brainstorm what kind of wishes Emily is likely to make. I am thinking destroy Nilbog is one. But I am also thinking she's so paranoid she probably thinks Grail-kun is parahuman and wants to avoid him as well- maybe wishing for something like "Go away". Idk.

So do you want to cause every single Parahuman in the world to have a Brain Aneurysm? Cause that's a pretty good way to do so.
What I want is irrelevant; the question is what Piggot would be willing to wish for. Given her personality, I can see this being her fondest wish. I'm not sure if she'd be willing to voice it, mind you - but I can imagine it.

If people think that, amidst the ongoing extinction level event, Emily Piggot will not seize and and every opportunity to save the world, you are deluded. "Give me power to save the world" is what I expect her to say. Or "stop the world from ending". Or "ensure survival of mankind". Stuff like that. Above all, she is a professional and she will use anything and everything to help carry out her duties.

The same person who would rather live with a crippling injury requiring ongoing, costly life support than allow Panacea to heal her?
Citation needed. As @Cyclone pointed out, Panacea doesn't do requests. Clockblocker's dad, who he would have absolutely pleaded about if he had opportunity, was still dying of leukemia in canon by the time Panacea went into voluntary Birdcage exile. Clockblocker has been a ward for what, at least a year? Piggot not being healed by Panacea is, at most, a very weak, tangential evidence of her not accepting help form parahumans.

Another possibility is to alter the species of every para-human such that they are no longer human, maybe upload all of them to their shards or apply a retrovirus that turns them into kryptonians, actually we could turn everyone into kryptonians.
