To me feel hot springs seem like the best place to be right now, we won't in the borders of a country actively hunting us. Spending time with our brothers ooc is a good way for us to get to know them, in character they're the only family left so, yeah. As for covering our tracks, we're actively getting hunted, I want to make it as hard as possible for our pursuers to find us.
 
Before we dive into the Land of Hot Springs, I think we should consider where each of the starting options are likely to push us and consider which of those final results best fit our goals.

Starting in the land of Earth puts us close to four minor Hidden Villages: Takigakure, Ishigakure, Amegakure and Kusagakure. Of these, we know very little about Ishigakure and a bit more about Takigakure and Kusagakure. Both Kusagakure and Takigakure are (in the anime) political allies of Konoha with powerful and important secrets of their own (depending on how much of filler/movie canon is being adapted, in Kusagakure's case). Takigakure is the only minor village home to a Jinchūriki and it's managed to protect that Jinchūriki throughout the generations, suggesting it has a competent understanding of informational security & a decent level of defenses. I am ignoring Amegakure, because it's as likely to bring death as staying in Iwagakure is. The flavor text suggests that Iwa thinks we are fleeing east, which would mean towards Takigakure, but we would have freedom to choose where we're fleeing towards.

The geography of the Land of Iron is less clear, as it isn't present on the primary Elemental Nations map. Unlike the Land of Earth, where staying in one place will mean certain death, the Land of Iron is probably a less immediately dangerous place. Although the narration tells us that "the samurai are after the blade we have," this is an area where I think we would have more room to argue with individual samurai than we would with individual shinobi of Iwa or Kumo, by appealing to the bonds those samurai have to their own blades.

The Land of Lightning is extremely dangerous. I think, of the options presented, it is the most likely to have us die immediately. Our chances to escape would be to make it to the coast and flee aboard a ship (which has a lot of complications) headed towards Water or Hot Water, or to make it out by land into the Land of Frost. The border with the Land of Frost is the Land of Lightning's only by land border in the usual Naruto map, so anyone hoping to find us will have a definite presence there. There is a potential positive in the Land of Lightning, in that it isn't specified whether the force hunting us is related to Kumogakure (in the Land of Earth, it is implied Iwagakure itself is hunting us).

The Land of Rice/Land of Sound is an interesting option. In the anime, it's home to the Shiin and Fūma Clans, who would later go on to join Otogakure and Orochimaru. These aren't a presence in the manga, however, and it's unclear what canon Magoose will be going for for this adventure. The setting blurb given for it implies that there are no ninja present inside of it, and while that may be true, it's also exactly what Orochimaru would want us to think if he is already present. The Land of Rice should border the Land of Hot Water and the Land of Fire, meaning that there should be good escape routes from it into more safe territories if things become too dangerous for us. However, if Orochimaru is already present, running is probably a lost cause.

Last, there's the land of Hot Water. Being in the Land of Hot Water would put us in the middle of the politics of three major factions immediately, as Konoha, Kiri and Kumo compete for control over the region. Of those factions, two will immediately hate us (Kumo and Kiri) and one will be suspicious at best. Yugakure is a decent Hidden Village to take shelter in, but we need to keep in mind its stated values of pacifism -- if a stronger force comes for us while we're taking shelter there, we're much more likely to be given over without a fight than we are to be protected by the village's chunin or jonin. Considering our proximity to Kirigakure in this option, we would have to consider the possibility of The Nameless Blade being one of the artifacts of The Seven Swordsmen of the Mist -- or the possibility of others thinking that it is one of the artifacts of the Seven Swordsmen. Starting in the land of Hot Water is the option that puts us closest to the ruins of Uzshiogakure, though, so we might be able to raid the ruins for something of value... if there aren't already enemy shinobi wandering the ruins in search of the same thing.

...anyway, with that in mind, I think starting in the Land of Earth or the Land of Rice provides us the most interesting immediate scenario and the most freedom in where we flee too. With just how many other Hidden Villages hate Iwagakure, being an enemy of it might prove to be an advantage when we make it out of the region. For the time being I'll put my vote towards a Land of Earth plan, but if that's too doomed I'll change to join meri47's Land of Rice plan.

Covering tracks seems useless right now-- we are staying in place until Shirou's sickness has passed, and we haven't recently done anything to draw attention. Training jutsu or taijutsu won't be enough to protect us against any actual enemy shinobi, but " The Blade will protect you ???" flavor text suggests that something about the Blade's secrets might. Teaching the heritage of the Uzumaki right now seems like a trap; Jin is brave, but an idiot, so any secrets he knows of the Uzumaki are likely to be shouted off at the first sign of combat. We will need to steal supplies in the near future, but doing it while Shirou is sick seems like a good way for Shirou to die.

This place was simple enough… a place with few ninja, and even fewer problems. You knew from the stories of your mother and father that it was a peaceful land, one where we could be safe for a while.

But even the promise of safety couldn't stop the hunter that was following you… hunting you like you were nothing more the cattle, or stray dogs needed to be put down.

There was no safety… but there was respite… You could sleep somewhere warm for a night… hopefully.

At least it was peaceful.

And there was plenty of food in the trash, uneaten food that was thrown out that was edible enough to eat.

As it turned out, once food got past the gag reflex, a whole variety of food opened up to you… not always healthy or tastey… but it was food.

You rubbed your eyes, taking a deep breath. "I'm okay Shirou… I just need you to get to sleep, okay, so you can rest."

Shirou had tears in his eyes. "I'm sorry Koto… I'm sorry I'm not strong like you."

You gave your brother a hug. "Try to get some sleep… think of the warm food we'll have tomorrow."

The prospect of something worse was on your mind… but he didn't need to know that.

Shirou was asleep as you placed the wet towels on his forehead. He had been sick since the previous week, a fever, and a sore throat.

He didn't tell you… That was stupid of him.

Didn't he know you were the one who needed… no, you had a duty to keep him safe. He won't survive too long if he can't get proper treatment or medicine… and you can't risk going to the hospital or doctor, for fear of being tracked by the hunter.

You hated yourself… you didn't want to lose him too.

Your two brothers were all you had left… you weren't going to bury them too.

Reward: Shirou is recovering… but he's weak. He'll need a doctor or medicine to survive the next coming days or constant care.

Jin was meditating quietly as you sat next to him. He was… preparing himself. While he did not have the sword or have a turn to watch, he was ready to do whatever he could to defend you.

You didn't want that… not now, not anytime soon.

"Jin." you said his name quietly.

He opened his eyes. "Yeah?" He asked.

"We need to talk." You said.

He huffed. "What is it Koto?" He was annoyed, but he gave you his undivided attention. He understood what you needed to say was important.

You gave a nod. "Jin, do you remember what mother said before she died?" The memory was still fresh in your mind.

And as it turned out… in Jin's as well. He gave a nod. "Remember what we are?" He stated.

You nodded. "What else?" There was one more thing he was missing.

"That we had to protect each other." He said. "But I am protecting us."

There were the magic words… and the heartbreaking statement that you did not want to hear.

"Do you know what that means?" You asked him.

"That I need to be brave, and strong." He replied.

You shook your head. "No… it means we have to make sure we do not make mistakes… we do not" you paused. "No… we cannot be caught. We need to stay safe."

Your brother understood the words… but you needed to be sure.

"There is a time and place for you to be brave and protect us Jin." You stated. "But it's not now."

Hopefully not ever.

Pretty much with our detection low and Jin convinced not to do anything stupid for the time being we have a chance to gather supplies in relative peace so we should take while we can. The problem with getting a doctor for me is that doctors are generally expensive and we have little to no money so unless we run into someone with a bleeding heart I don't think they'll help us so it might be better to just steal the medicine ourselves. We need to figure out the Blade and right now with no one actively chasing us we have a proper chance.

Aside from the plan, where are we going in the long term, what's our endgame here? Yusa might be a good enough ninja village but as Bolt pointed out with their pacifistic tendencies they might just give us away to our pursuers without a fight, not a good place to settle in my mind. To me there are two places we can try to settle down in.
  1. Somewhere so obscure that the people chasing us won't know where we are at all, a minor ninja village or just a small village in general, that offers us safety in anonymity. The problem with this is if our pursuers find us before we're strong enough to fend for ourselves, it's back on the road for us again.
  2. Somewhere strong enough to deter most of our pursuers, one of the main ninja villages or possibly the Land of Iron, this offers some immediate protection and we'd have strong people to train with, the problems with this are of the five major villages only two of them aren't actively hunting us and of those two they'd probably need some kind of incentive to take us in, we'd need to be able to offer them something.

The doctor is rather risky, leaving behind an obvious witness, but if Shirou's state is bad as it seems we better get a professional care and not poison our little brother with ill gotten medicines and lack of knowledge. Hopefully we could use henge well enough to cover our tracks.

Because we've chosen the Land of Hot Water, I don't think that safety in obscurity is going to be a very safe choice. If we can get out of the region, through Fire, to Grass or to Bear or to some other minor region, I think it would be a viable choice, but with our current location it seems like it wouldn't end well. If this wasn't a quest, and we were focused entirely on our survival, I think we could find safety in a small village, dye our hairs and work our way up from there in a relatively safe way; but I think that path would be one where we're practically giving up on our shinobi heritage in order to remain hidden.
