It is. Happened to Ichigo, and Kenpachi. It's just super rare because you need absurd spirit pressure, and everybody who has that much power generally long since learned the name. Also, very little control.I'm not sure that's even possible without knowing its name first.

Nope. Like, there is no ambiguity here, Kenpachi's sword is not released. You can tell this by the fact that it's a sword and not the gigantic axe that his Shikai is.Might be.
