Idea for Dr. M would be like Jason. She is Cauldron, but personally weak so like summons and boosts the members of Cauldron. That and/or be a Caster base and make an army of 'monsters'.
Interesting idea, but it does beg the question if Doc Mom would even qualify for the Throne.
It would have turned into a precog battle after a certain point. Archer!Gil has access to Caster!Gil's precog he just has such an ego he refuses to use it in most instances since it could show him losing.

Honestly, for contessa i'd just have some of their stuff be blindspots. Properly messes with her.
Okay, precog fight between Gil and Tessa does sound kinda fun...

The key factor is having an indelible legend embedded into humanity's collective consciousness, above anything else; OR the alternative scenario:

Unless Doctor Mother accepts an offer from the World (re: Alaya) to join the Counter-Force*, she would NOT qualify; as for the rest of Cauldron?

The Triumvirate would qualify, being known world-wide on at least Earth Bet, and Contessa might qualify as a Phantom Spirit, having an urban legend at most.

* - Which in turn would require her to have some talent Alaya deems worthwhile enough to join the ranks of its Guardians, much like EMIYA's Unlimited Blade Works and the counterfeit Noble Phantasms within, making him a budget Gilgamesh, or Kiritsugu's Time Alter magecraft, making him as a Guardian (lore-wise only; is nerf'd to hell in F/GO gameplay) a terrifyingly effective assassin.

They had a literal army of Case 53s that would know of her, and she would meet with all the vial buyers. She's the 'face' as it were of the group talked about in rumors and whispers.

There are Assassins that make it onto the Throne with similar.

Unless there are thousands of such Case 53s who survived Golden Morning and then spread the tales of 'Doctor Mother' far and wide, no... that is not how it works, IF strictly following Nasuverse rules.

There are Assassins that make it onto the Throne with similar.
Missing the point; the Order of the Hassashin itself, embodied as 'the Old Man of the Mountain' and his various unworthy successors is what the legends speak of, and almost never the actual individuals.

As to why their abilities end up being recorded in the Throne of Heroes, despite only having fewer tales to their names, such as the Hassan(s) of Serenity and - of the Hundred Faces? Chalk it up to Age of Heroes (not necessarily 'of Gods') BS.

I mean they had enough Case 53s to cover the world Cauldron's massive planet wide base thing was on. Does that not technically count as spreading her tale across the globe? Or a globe in any case.

If they took the opportunity to scream out the tale for any and all to listen (and was further spread by said listeners), that might be a viable scenario for a 'profile' in the Throne of Heroes.

Nonetheless, at best Doctor Mother is either combined into / with Contessa as a Phantom Spirit (likely a dual spirit origin), rather than a Heroic Spirit, and thus would be of minimal value as Servants.

Legend is everything when it comes to Noble Phantasms, which are what make or break Heroic Spirits (in a hypothetical versus, re: Grail Wars). Not their attributes or statistics, their Noble Phantasms.

Case-in-point: it isn't entirely impossible for modern-day magi to physically contend with Heroic Spirits, just requires an absolute unit of a modern human to pull that off, re: Arou Myourenji a.k.a. Scandinavia Pepperoncino... it's once the NPs are broken out that modern humans stand zero chance, barring cheats like EMIYA, since FEIKAAA.
