There's a vague implication that World of Darkness is post-post-post-apocalypse Creation, which would mean you could get there through sufficiently robust time travel.

Personally speaking, I never liked that implication, and even with crossovers between the two or with other franchises, I preferred the explanation of "Earth is actually Gaia's worldbody, so it's separate from Creation but still theoretically possible to travel there, if it still exists at this point."
