anything to do with the mad doctor is not the right thing, the fact that it lets us heal is the only benefit and we REALY need it fixed if we every want to fight again.
The Nothing option states it gives us the chance to ease Yammy's pain by making the torture armour less horrific. Also, we don't need to fix our injuries to fight; we're still capable of fighting at a decent level in the skills our crippling affected, and things like our Cero weren't touched. Not saying that it wouldn't be extremely nice, but it's not a necessity.

Hell, he's probably already filled us full of spy-bacteria so he can get more observational data of Cirucci and can keep track of our movements and find out what we get up to when nobody's watching (I suspect Nemo has something of a reputation for being sneaky now). Giving him free access to our insides is likely to end with a mix of beneficial, creepy, and potentially hazardous results- us having a permanent tracking device implanted and our lungs retrofitted so we can breath acid at people, for example.
Nah, spy bacteria is Mayuri's trick and Szayel isn't on Mayuri's level... yet.

I imagine he'd actually have to do something we'd recognize as medical to Nemo to install surveilance. And I imagine he really wants to. Why settle for just her assistance on one project when he can steal all her secrets by watching through her eyes whenever she works? He probably even realized she designed that weapon Cirucci used against Nnoitra (the eye) that turned the battle around, making him interested indeed in the potential of her work.

[X] Work with Szayel. In exchange you will ask for…
-[X] Healing your crippling injuries.
With the Quinta as your mistress, there is no risk of him offending her by turning you into a freaky mutant, but letting Szayel play around with your insides might still have consequences, even - especially - if he believes he's helping you doing so. Then again, these consequences could be positive…

So, as far differences go:
Aaroniero eats memories alongside his meals, and carries them with him (the song he sung probably came from one of Butcher King's servants).
Szayel twists the mind and throws it aside, often leaving his servants and tools mindless, in one way or another.

Aaroniero tasted Nemo's mind, and memories of those who met her. He knows her, as an old friend does.
Szayel sees Nemo's body, both her La Marana tools and her injuries. He understands her, like a veterinarian does an old cat.

Aaroniero sees the value in coming back to the roots, to consuming everything and growing wholly, holistically. He wishes to become more whole, better.
Szayel sees the a Hollow as a raw material to be molded into a more complex, more innovative shape. His works are tools for specific purposes.

Aaroniero appears monstrous, but he's actually a very nice person (besides being an apex predator, of course).
Szayel lacks any empathy at all, his smile and warmth is a habit, at best.

Aaroniero prefers the damp, dark caverns, organic in their layout.
Szayel prefers strict organization, even if it's twisted like his own mind.

Aaroniero's body is inhuman.
Szayel's mind is monstrous.

Aaroniero's a farmer.
Szayel's an engineer.

Depends on who we're fighting. I'd really rather have our injuries fixed before fighting a Vice Captain, say. On the other hand, I'd rather not have Syazel be the one to do it, since he'd almost certainly do some tampering and I don't want him watching through our eyes as we read the research we pocketed without telling Aizen and then having blackmail material.

It's a tough choice.

Szayel actually has something pretty similar to Mayuri's bacteria- he loaded his brother with covert spirit recorders (some sort of insect according to the wiki) before he died. It's how he knew enough about Renji to be able to nullify his bankai right off the bat.

The size difference between a bacteria and an insect is pretty immense. And we don't know what the loading process entailed. We probably won't just inhale his spy mechanism. Too bad we don't have a friendly medic to check us for parasite infection after this, he might have done something when he poked us, though, relying on the injury's pain to hide it.

-[X] Nothing. By feigning pure scientific interest in his work and an eagerness to help him, you will please him greatly and make him less likely to scrutinize your work, putting you in a better position to make the Sacrophagus less inhumane, and perhaps giving you the opportunity to pilfer Szayel's notes on other subjects for your own use.

The bottom Espada are both such creeps but I can't help but like them. I'd like them more if they weren't so threatening though...

I definitely want in on this, it's interesting and useful but I'm unsure what to ask for.

I kinda feel the Cirucci option is too presumptuous; stepping on her, his and Aizen's toes. Asking for nothing also feels like too unstable an investment. It could work out great but I don't want to bet on our ability to play Szayel. Maybe with healing out the way we'll have the chance to learn more later?

[x] Refuse. Nothing is worth the moral hazard of working on this project, or the dangers of Szayel's laboratory. You will offend the Espada, who might not even understand why you're letting go of such an opportunity, but Style will let you do so smoothly enough that he won't hold a grudge.

Shit. I just realized the real hidden cost of accepting Szayel's offer to work together. It's nor moral decay or letting a mad scientist do things to Nemo's insides.

It's giving Luppi more time alone with our new minions.

Insects do get small enough that you can't really see them without deliberately looking (less than half a millimeter long), and these things would almost certainly be designed to be hard to spot (so at the smaller end of the insect scale and maybe have some other means of camouflaging themselves).

I'd certainly like to have our injuries fixed, agreed. I'd just prefer to do the right thing more; it feels more IC for Nemo, and if we did let Szayel fix us I'd half expect us to wake up with our stomach altered so it can be ejected as an emergency explosive.

Accepting his healing seems like a real great way to get a bunch of horrific surprises jammed inside us.
Quite frankly, it feels worse even than outright never getting healed.

I'm voting for this because I have a feeling we'll see combat again relatively soon. If there are other options then this we can get in the near future I might change my mind, but I can't think of anything.

We don't have a way to keep it from being a torture device altogether, but we can make it less of one. If we were the ones who were choosing to strap him in the thing you'd be right, but that decision is out of our control. Because of that, in this situation reducing the amount of torture does matter- it's the only thing we can do to reduce the amount of pain Yammy will suffer. And doing something to help is better than doing nothing.

Getting healed by a mad scientist means becoming less of a person (cause helping make Iron Maiden Suit), and more of a weapon (cause of course he'll make us more combat ready).
It could be:
-Fusing with her La Marana creations, making them more potent, if permanent.
-Bringing out her shadowy inner self- good combat potential, but inhuman.
-Maybe use her HSR to... create biological weapons inside us, to eject at the enemy?

Second options heals the soul (cause mercy), improves the mind (cause notes).

Yea we finally got a chance to get healed it would be foolish to pass this chance up.

Compare the pain of a sliced finger to being flayed alive if you will. We can lessen the pain. I still think we should refuse, but the other options are fairly moral and in good standing.

Considering Cirucci's feelings on the matter of Yammy, might get us a few brownie points to know we helped him, even if only a little.
