this means North Korea is now an impoverished yet fully free democracy, while Sweden is a highly-developed authoritarian dictatorship.
The countries in grey stay the same because the Democracy Index has no ratings for them and Fred's too lazy to come up with something himself. Additionally, those with the yellow shade of "hybrid regime" stay the same as they were in the middle of the scale.

Basically every country in the world get's their history changed by ROB so that their democracy rating get's reversed. Example being that Canada now has a lower democracy rating than north korea in OTL becoming 0.92 on the democracy scale. Democracy Index - Wikipedia. Side tamgent:

I'll start things off by saying that canada becomes fucking ingsoc from 1984 in this Clusterfluck.
 
Not enough information. There were a lot of ways for history to go otherwise to leave America less democratic. All of them have severe implications for much of global geopolitics.
As such, I'm going to go for an interpretation that assumes the drop in democracy rating is a very recent phenomenon as a result of a hypothetical hyper-expansion of the power of the media.

Depending on various factors, this results in anything from a weird funhouse mirror of 1984-style "you believe what we say" that keeps an actually credible external veneer of democracy to [Second Redacted Comment].

Well, if by best you mean that it ends up being democratic and with a high standard of living, then you need a wealthy but highly unfree country to begin with.
Qatar has one of the world's highest GDP per capita rates, but is classified as an authoritarian regime by the Democracy Index, so that's probably a good example. In general, the Arabian Peninsula (except Yemen) would be the 'First World' in this scenario.
