Well since there's a nonzero chance Zenkichi actually met some of Dio's minions, he might have picked up something.

I think she doesn't know everything (though this is Medaka) but she probably knows enough to have a general idea of everything.
That's an understatement. His reaction to Zenkichi can be translated as "Did we miss a Special? No, did we miss an Abnormal?" I expect him to ask around about Zenkichi, which will possibly attract attention to him from various sources.

After. Let's just say that thanks to Zenkichi Sugar got out of a rather shitty situation. It's only recently that he came to Japan from America.
...so eithier Zenkichi traveled around a lot with his mother (it's not irresponsible when they both have Stands, including one healing one) or he went on some training trips with people who may or may not have been Dio's minions.

What about if Zenkichi is Dio's only son? Lot of subtext in that.
It's possible. However, according to the wiki Giorno in canon was born in 1985 - which is the year when Dio met Hitomi. In other words, he would have been concieved before that happened (unless there are more unseen nails and butterflies).
So Giorno is likely to exist, the other three are rather less certain.
