During the end of the second age the Elven realms of middle earth suddenly find themselves in the world of the Witcher in the lands of the northern kingdoms.

How do the Elven Lords and Kings handle deal with a world of common in magic and monsters along with not so friendly human kingdoms who have long since gain dominance over the worlds Elder races.

What do they say of the Aen Seidhe and if met the Aen Elle, that of the many monsters that stalk the night. Of the men who would ever so gladly attack them for simply being elves?

Bonus scenario: The dwarven holds, Shire lands and some Orcish tribes are sent along with the Elves.

They crush anything in the setting for the most part. They're all more superhuman than Geralt, the Lords produce crazy shit like Glorfindel who can chase away ringwraiths just by revealing himself and fight Balrogs one one one. One top of that, they have full use of their rings without needing to worry about the One.

They are also pretty self absorbed and isolationist, so they aren't likely to go conquering or anything.

I suspect that their biggest effect will be when at somepoint somebody inevitably decides that they can't really be all that dangerous since they aren't trying to conquer anybody, and sends an army either at them, or tries to march it though their territory. Then the elves have the trees eat them or something, and the local power structure gets thrown into chaos.

Would that be the end of the Second Age as in After Gil Galad's fall when there were mostly silvan realms and the tiny sanctuary of Rivendale or is the last High King still alive?

The elves desire to expand will very strongly hing on that I feel, also on the question if they can still pass into the undying lands. If they still can than they will eventually leave, just like from Middle Earth, perhaps leaving a few interesting stories for scholars to puzzle over, or maybe a little help to the local elves. If they can't well... they will make the most of their new lands and by make the most I mean pity anyone who stands in their way. Geladriel may yet recall why she wished to be queen in the lands of the Sun.
