Just in case we get trapped too or such.
Would you consider switching to talking to the professor first? You've always prioritized caution in this quest, I'd really rather we don't get caught unaware by something. Normally I'm not this risk averse, but the presence of the Necronomicon impies all sorts of horrors that could lead us to severe consequences if we fail rolls. We got so lucky with the painting. I really think we should go into this with an inkling of the nature of the threat. Even if the professor is uncooperative Emily at least will have insights.

[X] Go to the Asylum and talk to the professor if he's better and gently bring up Necronomicon.
-[X] Tell him there is a plague ravaging the city and he must help put a stop to it.
-[X] Tell him you know how dangerous it is and you must know what happened with his research. What did he stumble upon in his trip?
-[X] Hint about some of your experiences with the Supernatural and how you have dealt with threats before if you think it will put him at ease.
-[X] Follow up on Emily and the portrait after the conversation if nothing comes up.

Fair.

Not 100% on the sub votes, but the general thrust of "lets go see the professor again" seems good.

What He Knows
You returned once more to the state asylum to visit Professor Herbert Cowan. He looked better than he had last time, if with a restlessness that seemed to keep him from entirely relaxing. He smiled at your entrance and beckoned you to take a seat and visit with him.

"Hello professor. Remember me?" you asked.

"I do, if not how you came to decide to visit me," he answered joyfully. "Quite curious. I don't believe you are a former student of mine?"

"I am not. Simply someone interested in your work."

"Well, I'm sorry that you came to find me in such a sorry state."

"That's alright, I understand the dangers that the occult can hold," you said kindly. "A find like the necronomicon must be especially dangerous to study."

"Indeed. If only I got further in my studies."

"Far enough for problems," you replied. "Professor, there's a plague affecting Topsfield."

"There is?" The man sat up in alarm. "Oh dear, that's terrible news."

"I believe it can be fixed. I just need to know what you did."

"Me?"

"Yes." Professor Herbert still looked confused. "Your research with the necronomicon?"

"I haven't studied that in years. I transcribed some pages from the copy I found, but alas it never went as far as I wished it to."

Your heart sank. "You haven't read it recently? Done anything?"

"I've spent the past few months held prisoner in my home as something hunted me. Before then I was quite happy in retirement." He shook his head. "I'm afraid I know nothing of the plague you speak about."

You left the asylum with a troubled heart. You crossed through to your home and looked for Emily. The poet was dancing silently in a room to something only she could hear. You waited for the right moment to step in and asked what she thought of the painting you sent. You explained to her what you had encountered. She informed you that the portrait's ability was powerful, but that there wasn't any connection there to the plague that you described. It simply acted as a way to call on creatures beyond this world.

[X] Make preparations to visit the Professor's home
-[X] Create a backdoor to the Red realm that you can escape with
-[X] Place an arsenal of tools in a secure place at Emily's that are in easy reach..
-[X] A lantern, sledgehammer, ropes, gunpowder, oil, honey, animal blood, religious icons, anointed water and food to use as bait. Painting supplies that you cannot easily carry. Remember to pack first-aid too and ammo for your gun. A supply of water you can use to breathe on from using Nemo's tattoos in case of miasma.
-[X] Prepare Water of Forgiveness, ready to unbind yourself from influence. Earplugs to stop yourself from listening to chants.
-[X] Prepare an elaborate ward of Pachamama's Sigils and another of Isis that you can use as shield depending on the type of Foe, Eldritch or Undead. Long sheets of protective sigils that you can paste on surfaces. If they are too bulky to carry leave them at Emily's to retrieve as you need them.

I don't know if this will be enough but these what I can think off the top of my head, for an unknown threat.. Do you guys have more ideas? Feel free to add or remove subvotes if there's something you think we need or don't need.

Looks like it might be something that followed him from the cave-in instead. We could perhaps follow up on that?

India.. rakshasha? I also remember chanting mentioned, that might be relevant. I'm adding earplugs just in case.

Edit:

Hmm. There feels like there is an inconsistency we need to figure out here.

So I like being prepared, but there is the time factor to consider. I would not want to spend more then a few hours. Every day may have consequences. So nothing elaborate.
Don't we already have a door to the red realm?

Anyway, checking the neighbors seems like the best bet to me. Should tell us the plague and the professor situation are really related or if it's just a coincidence.

Do you think we should take the risk and check his notes before entering the house?

Can we at least bring the earplugs though? The chanting was mentioned by the professor and it could be the thing that traps.

I know we have a portal but I mentioned it to have the prepwork done for posterity and keeping the escape clause. The rest of it is mundane shopping and organising. We could scrap the ward making in the last step as that would take too much time.

Edit: I really don't think it's worth it to mess with the Necronomicon notes any further than we already have. The chance of Oliver being 'altered' is terrifying.

Yeah checking the notes is probably too dangerous.

Neighborhood
At this point you can only think of looking closer to the source itself. You head down to the professor's home with ear plugs and a bottle of Water of Forgiveness. You feel on edge being this close. You expect you'll pick up on whatever is happening the closer you get. You stop first to check on the neighbors. All of them are sick. The professor's next door neighbor is in even worse shape. He seems like he's in a waking fever dream as he opens the door and looks at you. "Herbert? Here to talk more?"

[X] Introduce yourself as a friend of Professor Herbert Cowan here on an errand in his home
-[X] Ask him what they talked about. Was there anything strange about Herbert?
-[X] How long has they been sick? When did it all start?
-[X] Offer to do chores and use it as pretext to investigate their home
-[X] Try using the Water of Forgiveness on the sickly discreetly

This is an opportunity to see if using it on ourself would actually work. Looks like the plague is connected after all.

I wonder if there's a heavy miasma or something we could directly breathe in the water of forgiveness through Nemo's Gills to recover faster..
